Attempted Solution:

Barney came home to find his wife, Greta, in bed with another man.

Barney freaked out and Greta and the other man fled the house.

Barney, in despair, wrote a suicide note and grabbed a knife to kill himself with.

Meanwhile, outside, Greta convinced the "other man" to go back inside and kill Barney.

The "other man" reentered the house and surprised Barney from behind, leaping onto him and grasping his neck in a death-choke.

As Barney's vision faded he managed to ram the knife backwards and upwards into the "other man"'s chest.

The knife struck true, piercing the "other man"'s heart, but this only caused his hands to lock down more forcibly on Barney's neck, sealing both their fates.

Don't think your solution fits, it's been established that the other guy wasn't there to kill Barney and that he didn't meant to kill him.

Mandle's solution seems quite close to me , except that none of the victims got stabbed.

EDIT: Was the second victim's death rather accidental than intended by Barney? Obsolete,

Yeah, it seems to me that Barney strangled the other guy in a fit of rage and then committed suicide after realizing what he had done. But I'm not sure about the rest of the context... Why was the other guy there? Did Barney walk in on them? Or did he come over to tell Barney that Greta was leaving him? Where's Greta? I think we have the "what" but we're missing something...

I'm not sure if Barney committed suicide after realising what he had done, as the suicide note was written BEFORE either of them died. Other than the fact that we know that no-one was stabbed, I think Mandle's guess is a pretty good one!
