The online version is outdated:

You can also have a custom icon for the Setup program generated. To do so, create your icon as above but name it setup.ico in the game folder.

Just provide a valid icon in your game directory and name it user.ico.
It must be an ico file, you cannot rename a bitmap or similar. A good program to make custom icons is IcoFX, which is a free download.
