I don't get it to make a background

Okay this question is maybe for the 10.000th time here. 

I just don't get it to make an background...
I want to develope a game with a more serious storyline. So the graphics shouldn't be in an strong cartoonish style.
And the tutorial here isn't very helpfully.

Have you some professional hint to make an more realistic backgound?
