Can anyone tell how to make Dialog with different GUIs like second attache picture? At template I have simple square in the bottom (first picture).

I think the Custom Dialog GUI module can pretty much accomplish what you're looking to do. I haven't actually used it though, so you may want to read through that module's thread for more info on setup and use.
