You're just trying to get to the next screen by sticking to the left wall. Walk down off the bottom of the screen. It's best to take relatively short steps, and only when the room is dark (not flashing, but dark). Basically, what you don't want to do is move while the lights are on or flashing.

I've injected myself with the drug, done the brain-chart bit, looked through the computer, and I've been shot. So, assuming that there's a not-being-shot alternative, what am I supposed to do? I assume I have to find a way to get my "eye" guage to raise, but I've no idea how to go about doing that.

Save/load/quit: On the right side of the inventory GUI are two little buttons. The one on the top opens save/load GUI, the one at the bottom the quit one. If you want to load the game before the intro is played, push F5. Btw, all this information could have been found in the readme and/or the info screen you get when you press F12 in the game.

Mitchell's office is on the way to Knox's office. If you want to know where it is exactly, read the next spoiler. Otherwise just try and look at the doors on the way to Knox's office. If you find the right one, you'll get a message.

Mitchell's office is in the corridor where you can't go straight on because of guards, but just to the left to Knox's office. There is only this one door that leads to Mitchell's office. I think you still need to look at it or use the brain on it, to actually notice that it is Mitchell's office, as it now belongs to someone else.

As far as I remember you have to use the eye or the brain or both (I could check if you need to know precisely) on yourself and that's it. I didn't have any clue about the toe-nails before I used the eye and the brain on the player character and he suddenly used them to free himself. Just toy around with those and I'm sure you'll get the solution.

Yes, there is an alternative not being shot-ending and you are right, you have to get your eye gauge to raise. Usually this is done by injecting the drug in Mitchell's office. If you have done that, I have no idea whatsoever why you still didn't get that happy-ending, because for me, that just did the trick. Maybe I'm missing something or you might have found an unspotted bug here. I'd advise you to try again and if that doesn't work, maybe pm ProgZ. Good luck anyway.

Can someone please set me straight on this alternate ending? I can't seem to avoid being shot...I injected the drug in Mitchell's office, then I went through Knox's office, and through the computer, but he shoots me. If what I read in the previous replies is true, then I'm supposed to get my eye gauge full to avoid being shot, but how do I do that? What other information do I need? Alright, I think I've finally worked out how this game works:
How to get the happy ending:

It's not just about taking the drug in Mitchell's office as this is just one of the events that trigger a memory and therefore raise the bar. To get the happy ending you need to find all the lost memories in the game. Make sure to look at everything while you walk around the sanitarium and don't forget to also use your brain. I think that there are only three memories that can be avoided in the game. Read the next spoiler if you don't wanna do the searching yourself.

Make sure you looked at and thought about the following things before entering Knox's office:
1. the brain charts in the treatment room
2. the sign behind Burke and Hare
and make sure to inject yourself the drug in Mitchell's office. I just tried it once again doing all these things and got the happy ending, so I'm pretty sure this is going to work.

I didn't use the brain as much as I probably would have liked, either....are the remaining "catalysts", as I call them, difficult to find, or are they fairly apparent?

In the second part where you see Dr. Mitchell's "ghost", no dialogue appears. The apparitions just fade. Is this a glitch I'm experiencing, or is it supposed to be like that? Also, I contemplated the brain chart in the treatment room, and Noah identified each lobe and the specific Frontal Lobe picture, but is there something more to it? Also, the sign behind Burke and Hare just seems "familiar", Noah can never quite seem to place it. Is there something I'm doing wrong? Thanks!

YES!!! I found all the catalysts without having to look at the hints, and I found out what really happened! I have got to say, I did not see that ending coming. This was a very well-written game plot-wise, and very captivating graphics-wise. This is one of the best adventure games I've played, rivaling Yahtzee's classic 5 Days A Stranger. Extremely well done!

As a result, the ghost memory sequences flick on and off and you miss dialog that while not necessary does add something to the story. I apologize if this is an inconvenience. When I figure out why exactly it occurs I'll make an updated release.

I've played this scenario a couple of times now and never got dialogue, but just two people appearing on the chairs and then disappearing again. I always wondered why that helped Noah to uncover his past and find out the truth, but I always assumed that it was just something I didn't understand without suspecting the game to hide one of those precious cut-scenes from me...

Help! I can't get past the guard by the soda machine. Also, I can't figure out how to get a soda, which I think the two docs want along with the ciggs.

I just tried your advice, Venus, and still can't get it to work. The eye and brain on the poster in the rec room give me nothing but "It looks familiar, but you don't know why" type of messages. The posters in Knox's office give me nothing after the notice of the temporal lobe. I can't get back to the original lab at all (if you meant that brain poster) because it just says "I don't think Dr. Mitchell's office is that way". So, basically, am I just screwed here?
Going to keep fiddling around with this. If I figure it out I'll post the solution here. If someone else knows how to fix this or figures it out, PLEASE tell me since I am dying to know the rest of the story--this is an amazingly cool plotline...
I don't understand the logic of the drugs in this story:

This Noah guy was getting shots in order to wipe out his memory. Now he uses the same drug (in Mitchell's office) to restore his memory?! I don't get it.

When Noah halucinated (twice: once in the lab, then in Mitchell's office), he saw two persons: he saw Mitchell talking to some other person. That other person was supposed to be Noah, but, as we later learn, Noah and Mitchell are one and the same! so....???

Every time I use the brain on the cabinet the game tells me, that there is only lithium etc. inside, and then closes the cabinet again. How do I get the lithium?

First, exhaust all dialogs. The sequence of events seems to be important, so if you leave a bit, you get stuck elsewhere. If you can't seem to get the lithium even after a few attempts then you forgot to do something else in another room.
I opened the cabinet, then I searched the hotspots with the eye cursor.
Use the brain cursor and check your inventory afterwards. I could not get the vial before I sneaked past the reading warden.

I looked at (and used the brain) on the two brain charts in the room where you get drugged.
I talked to everybody until they repeated themselves.
I used the drug in Mitchell's office.
I used the brain on the panel behind the two guys that want cigarettes. Noah says the words are familiar to him but he forgot their meaning.
I looked at both pictures in Mitchell's and Knox's office, deducing that they knew each other.
I looked at the pcitures of the temporal lobe to get the computer's password.
And I'm still getting the bad ending. What am I missing?

I have done pretty much the same as Lufia, but looking at all the stuff in the "madness" sequence in Mitchell's office, but still get the bad ending.
I'm stuck right near the beginning, after I knock out reynolds or w/e and put him in my cell, there's a bunch of communicator panels with other cells that give no responses and a reinforced glass office with security equipment inside... All I could figure out was to eat the food ::)
Simple, just move downwards from the screen with the security office.
I really, REALLY hate giving out hints to my games because I feel that ultimately it ruins the experience for the player (and the joy of you solving it for yourself). There is a bug in that even though I got the bad ending, I played it again From the begining and did the same exact things and got the good ending.

Turns out that when I do all those things to get the memories, my memory bar is almost full, but I cannot seem to fill it, and still get the "bad" ending, perhaps I have fallen afoul of a bug? In which case, does anyone have a savegame for in Knox's office, with a full memory bar?
My testers and I went through this game many, many times (and so have I) and we have never had a problem getting the best ending with the right amount of points. Go back through, read the point locations carefully, and make SURE you have them all. Just looking at something doesn't guarantee points, you have to think about it and sometimes corellate between more than one object.
I just played through it again following the directions I gave you and got the best ending. If you are patient and thorough you will get the best ending!

I did this all (including the two in the corridor with the flickering lights) and still get the bad ending. Though I think I know where the problem is: I don't get the points for thinking about the two charts. I there a time when I should do it? Because when I do it while I'm attached to the bed, the eye in the bar remains closed and I don't get anything. Should I try to progress in the game and only remember the charts when I go back for the Lithium, for example?

Same here, I just replayed through the entire game again, making sure to look and think all of those points until you remember something, but I noticed that sometimes the Memory bar does not progress once you get the memory. I think there might be a bug that depends on which order you get the memories, because I only ended up with an almost full bar again.
It's no bug, trust me on this :) . The bar only shows progress after a certain amount of points have been accrued. The chart doesn't give enough on it's own to increment it.

1. Make sure you have downloaded the latest version of the game, version 1.2b. If you are unsure, redownload it from the first link.
2. Use Noah's look skill in conjunction with his thinking skill for everything; sometimes you may only figure out part of something so you'll need to look at it again. Unless Noah EXPLICITLY says he's figured something out (the two charts in the treatment lab are an example where he will explicitly say he knows what they are), you've still got some digging to do.
Keep at it, and stop seeing phantom bugs! You're making me nervous. ;D
No, I have examined and THOUGHT all the things you said trigger memories, with Noah explicitly mentioning figuring something out each time, I have version 12b, what could the problem be?
I had the same problem as you, Monkeyman, with the bar ending almost full. Quite frustrating, really.

Try looking at all the plague on the offices in the hallways (like Dr. Blair's, in the screen next to Knox's office door, the one with the notice-board). Try using the brain on it. Noah then asks himself 'What's a pathologist doing in a sanitarium?' Think about all the plagues in all the hallways. When you now click on the eye in your inv, Noah says 'Certain areas seem strangely wrong for a sanitarium'. That might be the last bit...
I also did all the other things Lufia mentioned, and I got the good ending....

I have all those messages, the only thing that seems amiss is that that the eye meter is not quite full, even though I have examined everything mentioned, I reckon there is probably a bug with the eye meter, depending on what order you get the memories, sometimes when I got the memories the eye meter DID NOT increase. If anyone has a savegame in the office with a full meter, could they put it up somewhere on the net as file please?

Try hacking into Knox's computer without having taken the shot of the Mind's Eye drug beforehand to get the 'bad' ending....
Although I have to admit, the posts in this thread were somewhat misleading on the 'getting shot by Knox' part. There is no ending where he doesn't shoot you (at least none that I know of).

I've managed to get past the corridor with the flickering lights and have given those guys the ciggies they wanted. I can get passed the guy that is reading but the only object I have is a now empty bowl and I can't get back to the treatment room to get the lithium.
I'm grateful for any hints as to what to do next.

Hm i think you have to talk to the 2 guards in the recroom before being able to take the lithium. I could not take it from the Cabinet before i got back from the recroom after i gave the cigarettes to the guards. Maybe you even have to talk a little more intensely to the other patients there.

I have Noah inject himself withthe drug in Dr Mitchells old office and I see the apparitions appear at the desk but then they disappear and everything continues like something important happened yet I didn't see any dialogue between the people in the memory. Was I supposed to? If so how do I fix it?
rippa32, I think I had the same problem. There is supposed to be a dialogue. Are you sure you have the newest version of the game?

When back in the cell with the gruel, you can pick it up, but I don't think you'll need it. At least, I didn't find a use for it. You can eat it if you want to but I don't think it makes any difference if you do.
If I remember correctly, you should try using the door.

I've finished the game, so if anybody needs help just ask for it, but don't give up!! it's easier than it seems at first, really. and it's quite short too, so if I were you I would go till the end, there can be 2 posibilitys, choose the good one o the bad one... !! :)

The hand is for taking only, the brain interactions/thoughts. It's not inconsistent, just not heavily used since there isn't much inventory.

Ok.. I can't get the lithium no matter what I do. What am I missing? I have replayed it several times.
This is what I have done so far:
1. While I am tied to the bed I:
a) look at the posters.. am I supposed to wait until later?
(and yes I did click on both of them with the brain and then with the eye)
b) look at the one-way mirror and intercom
2. While I am in the room the 2nd time under the influence I:
a) look at the figure behind the mirror
b) look at the dudes talking
c) put my hand thru them..
What am I missing?

If you are where I think you are, you've just discovered the book missing and that there's a Dr. Mitchell who might be able to help you. Try leaving the room and continuing down the hall to the rec room and talk to everyone there about everything :).

There are certain events you can pass over that cannot be revisited, just like real life. The good and bad endings are very subjective anyway, but once you get to Knox's office it's endgame. If you want the other ending you'll have to start over if you don't have another save.

Just got past that guard you need to sneak past, the one reading the book. I only have gruel, and am not entirely sure where i have to go. I gave those guys the fags, and got that guy his hat, but now im completely stuck. every room i want to go into seems to be locked and every other room noah won't enter anyway... so, any thoughts?

It's high time someone wrote a proper walkthrough for this game. Oh look, here's one!
You can pretty much find all of the solutions for the Mind’s Eye puzzles floating around somewhere online, (like this forum) but I’ve yet to see anything that really qualifies as a real walkthrough for this great game. I know a lot of people prefer this format, myself included, and if you ask me, a walkthrough for Mind’s Eye is a long time coming. So here you go.
There are two endings for this game, and even though one is really more dying than solving the game, I’ll go through them both.
We start our game in a small padded cell. Someone, who we later identify as Roland, is talking to us from outside. Talk to him as much or little as you please, this information isn’t really that pertinent. Once he’s gone, we can look around our cell a little.
This game uses a slightly different set of controls for interacting with the environment; in addition to the hands (use), eyes, (look at), mouth (talk to) and foot (walk) options we’ve got a brain, which functions both as an extra option to think about or remember things, and a bit like an extra “use” control. It’s a bit confusing to get the hang of what needs to be used where, but I’d recommend using it in tandem with the “look” option to get information about things. And to open doors, for some reason. Your inventory is that small bar in the bottom left corner, with the save and quit options being left and right bars (respectively) on the side of it. The bar on the top of the inventory tracks your points with an increasing amount of red liquid.
Anyway, look around the cell as much (or as little) as you like. It’s pretty clear we’ve got to find a way to get out of here, but with no inventory and the fact that we’re bound makes this first puzzle a bit frustrating. No worries. Look at yourself a bit, and notice your extra-long toenails. Apparently these are also pretty sharp, because if you try thinking about this a bit (use the brain on yourself) you can cut the straps on your straitjacket. Then we get a little cutscene and introduce Knox and Childs. You tussle with Knox, and end up tied to a gurney in the lab.
Knox exits the room at the end of the cutscene, leaving behind a book on the sink that you want to take a look at. But you’re tied up to the table, and still can’t move… or can you? All right, yes, you can. Basically by clicking around the sink a bunch of times with the walk option will get you to inch over on your gurney closer to it. It takes a while, and you do actually need to aim for the book, but eventually you can pick it up with the use button. The book doesn’t help you much, and you quickly pass out.
Back to the padded cell. You free yourself from the straitjacket again during a cutscene, then start hallucinating. Use the hands on the bowl of food on the ground to pick it up, and use it on yourself to stop the hallucination. Use the brain on the door open it, then walk out of the cell.
Walk through the first two hallways until you get to the hall next to the security room. Roland’s watching a security feed. Walk towards him. Childs will come and rough you up during a cutscene, but you manage to free yourself again by attacking Roland upon waking and locking him in your cell. With the security room now unguarded, you can proceed to Knox’s lab. Pass the security room and the lab will be the second door down in the hallway. Use the brain to enter the lab. A cutscene ensues, and your new objective is revealed: find Dr. Mitchell. Explore the lab a bit if you want, then exit the room. Make sure to save before you head south to the next room.
You’ll run right into a couple of doctors guarding the hall, and one of the most difficult puzzles of the game. Figuring it out is not too difficult, but executing it is another matter, and you’re pretty sure to get killed by Childs a couple times before succeeding.
The object will be revealed to you if you use the eyes and brain on the guards: you’ve got to try to slip past them by hugging the opposite wall and only moving forward a little at a time, when the lights have flickered out. If you get caught, they call Childs who comes and kills you with his billy club… not such a great idea. (Pressing escape will let you skip this cutscene if you get tired of watching it over and over). At least the game is programmed to automatically restore your last saved game after this happens.
Once you do make it past them, continue down the hallway to the rec room, noting the storage room on the right side of the hallway just before it. After you enter the rec room, orderlies Burke and Hare will offer to swap you some drugs for cigarettes. (Americans who watch this scene, rest assured that when they ask you to get them some nice fags, they mean cigarettes. They’re not planning on molesting any of the patients, at least not in this game.) Since you seem to think getting another hit of Knox’s drug will jog your memories about Dr. Mitchell, it appears that getting the cigarettes is your latest objective.
Talk to Lena, the girl in purple, about getting the cigarettes. She doesn’t have them, but she will draw your attention to the sad looking guy in the corner, Curtis, who’s apparently missing his cowboy hat and seems to be a likely candidate as someone who might have some. Ask Burke and Hare about the hat, and they’ll give you the ever-so-cryptic clue that they put it with “restraints.” They’re talking about straitjackets, so head back over to the supply closet just outside the rec room and use the brain on the black shelf against the left wall to get the hat. Head back to the rec room and give it to Curtis, then ask him for some cigarettes. He’ll gladly hand them over; pass them on to Burke and Hare. They want something else before they hand over the drugs you want, but they will relinquish some info on the mysterious Dr. Mitchell if you keep talking to them. Talk to Lena after this and she’ll point you down to the offices past Burke and Hare. Head off in this direction and enjoy the cutscene… it seems that Knox and the orderlies are on to you.
Once the cutscene ends, you can continue on down the hall and explore around this area a bit, but in order to really proceed you’ve got to go back to Knox’s lab. Use your brain on the medicine cabinet to pick up some lithium, and then head back past the rec room to see John, the guy who has to pee. Keep going (he and the room he’s in serve no purpose to the game as far as I can tell.) You’ll find yourself outside another security office, guarded by another orderly. Use the brain on the coke machine for an idea to get past the guy: hide behind the machine until he’s looking down reading (wait for him to read for about five seconds straight and then try it, this usually means he’s starting his “long read” and you’ll have the chance to sneak over there without him seeing) then spike his coffee with the lithium. (You can also merely sneak past him to the hallways past the security office, but you will eventually need his keys.) When he looks up he’ll send you back to the previous room, and another cut scene will start.
After the cutscene, head back to the room with security office, then go past it into the next hallway. The door to the security office is the first on the right. Use the brain to enter it, and then use the brain on the security guard to check his vitals and swipe his keys.
Here you have all you need to get the first ending, the lousy one where you don’t solve the mystery. Head down the hall and keep going until you get to a dead end and Knox’s office. His laptop is of interest, but you can’t log on until you know the right password. Use the brain on the projection screen at the back of the room to uncover a series of four posters. Use the eye and then the brain on each of them to reveal that Knox is a studier of the temporal lobe, also unlocking the option to try the word “temporal” as a password on the laptop. This works, and you’ll find an article on the laptop that tells of Mitchell’s mysterious death. Knox will enter and interrupt you, shooting you to death in the conflict that ensues.
If you want the better ending, instead of using the keys on Knox’s office first, find Dr. Mitchell’s old office, the one labeled for Dr. West. Use the brain on it to enter.
Explore the room as much as you want, but what you’re really interested in is the booze on the desk. Use the hand to pick it up, then head back to Burke and Hare to swap it for your drugs, then run back to Mitchell’s old office to use the drug and see if it jogs and important memories. It does, but this isn’t just a cutscene. You’ve got to look at the pictures hanging on the wall during the flashback, especially the one of Knox and Mitchell standing outside the hospital. After the flashback, you’ve got to find your patient file to solve the mystery once and for all. Head outside and down the hallway until you come to Knox’s office.
Now you proceed in the same way as you would for the first ending: use the brain on the projector screen, the eye and the brain on the four posters underneath it. Then look at the picture hanging next to the posters: the same as the on in the flashback you had in Mitchell’s office. Now use the word “temporal” to unlock the laptop, and enjoy the cutscenes that follow and the revelation of the mystery.
To get full points on the game instead of just finishing it, you've got to do a couple extra things. ProgZMax already mentioned them:
"Looking/thinking about both brain charts in the treatment lab, looking/thinking about the flickering overhead lights when they are on, looking/thinking about the sign on doctor Fuchs' door in the flickering room puzzle, looking/thinking about the plaque behind Burke and Hare, using the mind's eye drug in Dr. West's office, looking at the picture that appears in Dr. West's office when you use the drug and THEN looking at it in Knox's office, studying all the diagrams in Knox's office."
