get something from behind the crack under the cell wall?

I couldn't figure out how to do much in this game. I found the

bath spirits and sedatives
I went to sleep twice and had the same dream twice. I read the letter and book

I ended up overdosing and ending the game that way
I must be missing something obvious because there aren't that many hotspots to interact with or rooms that I see beyond that.
