Was the first knife supposed to go over her head and because of her wearing high(er) heels instead of the usual shoes becoming taller ... and therefor the knife hit her forehead?

Ok, I have a theory: The assistant was blindfolded and the knife thrower probably too. There was supposed to be some cue to tell her when she was allowed to move again, I'd think the circus band playing a jingle or something like that. It didn't happen as it was supposed to and she moved too early.

Would it have ended differently if she was male? (We are living in modern times but I in this historic event I am pretty sure of that)

So, she was tied to the wheel and started spinning, the knife-thrower put on his blindfold and started throwing the knives, but the assistant panicked and started struggling, which caused the wheel to slow down and stop. The audience started yelling to warn the blindfolded knife-thrower but they were too late and he threw a knife that fatally wounded her.
