Or better examples, how "Germany" once refered to all of German speaking Europe, "India" to all of Britain's Indian possessions and "Australia" to what is now called Australasia. The terms became attatched to the first or largest federations in the region, so the likes of Austria, Bangladesh and New Zealand no longer fall under the term.

This means there's resistance to allowing a great deal of routine tasks that require skill, but not Doctor level skill, to be conducted by paramedics and to a lesser extent nurses.
