Its a cool image, but I think that the guy looks out of place, the bottom half of him at least looks rather 'cartoony', where-as the skulls and candles etc look more realistic.

Darth, the paintings in the ceiling totally rocked. It improved this image by 62%.

Blurring the penis is a bit awkward, though. I suggest covering it up with a blanket.

Looking at the perspective, I think his legs should be bigger/thicker. The table is getting wide, while the legs stay more or less the same size. I hope you know what I mean

Maybe there should be some symmetry on the ceiling. I mean, this guy's dying a horrible death, at least give him something pretty to look at.
And there's no reason not to have a penis in your game because we are independant and are, as such...ABOVE THE LAW!

Wow, the room is very immaculate. I suggest extending the images of hell to the walls. The added detail might make the room seem more claustrophobic(sp?) and thus more menacing.  The tile floor looks to be in good condition. Perhpas some cracked or even missing corners of linoleum (or asbestos, if the tiles are as evil as the room itself.) The stone pillars look old, so the new tiling looks a bit out of place. The table should be remain clean and polished, though. I know if I had a table for offerings to Satan, I'd keep it clean so I can see the organs and what-not mirrored perfectly in a mandala of orgasmic pain.

There's so much detail on the shelves that they distract from the overall image.  I might drape a cloth over his nether-regions, less for obscuring the penis The Penis and more to add interesting details.  His limbs are at very stiff/straight angles and that makes him look more fake than skin tones or other small problems. 

I'd also expect that unless that's where he's been laid to rest, there might be manacles or leather straps holding him in place.  A blood gutter also seems appropriate under the circumstances.  For the painting on the ceiling, I'd rather see vague/blurry details than something that looks fairly cartoony (mostly the demon/dragon's expression, the people seem alright). 

I think the dragon could be fixed by removing the pupil and putting some structure lines around the eye to give it shape, there's just something about the wide-eyed expression that makes it comical.

It's good work, makes me wish it were for a game rather than an album cover, but *shrug*

Nice and freaky. If there's one thing I don't like is the art on the ceiling, try drawing it as you would on a flat surface and then skew it to fit the ceiling.
