It has to do with the handcuffs which seems to represent letters and even a heart. So all the endings are represented by the handcuffs or the "S" represents that there's six endings or both of these. Something like this.

So you have to give him somethings else what he'll accept
The placebo is fine for the moment, but you have to make it look like his medicine
