It is the nailbrush that you need. Maybe a bit of a pixelhunt but you need to pick it up. Did you have a look at the ID card to realise you need the moustache?

?? When you give him the cigarettes you get the key... (at least I thought...). You can take the smock from the room with the glass cabinet. You can use the key on the glass cabinet to open it.

Nevermind, I figured it out. I had to:

Cut the bristles off the brush. And then get a tube of glue out the cabinet to make the fake mustache.
